What types of information does Learn.net collect and how will
it be used?
Learn.net only collects information on a voluntary basis. The
personal information Learn.net collects is used almost exclusively for tracking
and reporting training activity for and on behalf of our customers. Only information requested and approved by
our customers is collected from visitors to areas specific to that
customer. The information collected
usually will take the form of:: (1) MAPLE learner registration
form; (3) information regarding the usage (time, and location) of educational
content (4) Course surveys; (5) Learn.net
Information and Request Form; (8) general information contained in an email
(e.g., comments). The types of information that may be requested for each
activity and the way the information may be used are detailed below.

MAPLE learner registration form
Visitors that wish to enroll in online training and/or register for educational
sessions presented by our customers are required to complete MAPLE learner registration
form. The requested information may include the student name, address,
telephone number, name of agency/organization the learner is affiliated with,
membership information, email address.
In
some situations, customers of Learn.net may request that demographic information
be collected. Usually this information includes
gender, years of experience in the industry, and date of hire.
In
some cases the educational courses and activities provided by our application
are eligible for filing with state regulatory bodies for Continuing
Education. The information requested as
part of the registration form is specific to the state of residence and/or the
state of filing the CE credit information and varies by state. This information may include License
number(s), Social Security Number.

Information regarding the usage of educational content and
activities
Tracking the interest and access of educational content and activities is
the primary function of the MAPLE application which is at the core of many of
our web sites. This information is
collected on behalf of the customer and is available for their sole use. This information usually includes, date and
time of each access, length of access, preferences, selections, and other
end-user settings. Additionally, some
online content contains the ability to “bookmark” progress and as a result
activity within courses may be stored in this capacity in our application. Test scores and responses to test questions
are also recorded for the purpose of grading, test item analysis, and review.

Course Surveys
From time to time, customers may author and present course surveys or other
feedback mechanisms to the visitors to their area of our websites. These surveys are authored entirely by the
customer. The access and responses to
these surveys are recorded and collected.
These responses are available to the customer that authored the survey
for their purposes.

Anyone who wish to have access to our application or additional
information regarding our products and services will be asked to submit their
company name, the number of stores operated by such company, and contact
information such as contact name, mailing address, phone number, and e-mail
address. This information will be stored for purposes of developing a
relationship with potential customers and helping them educate their potential
student communities about Learn.net and the products and services we provide.
